CASE MANAGER

The Tulane University Center for Brain Health (TUCBH) is a clinical care and research center for Traumatic Brain Injury (TBI), Physiological conditions, and Behavioral Health (BH) conditions such as post-traumatic stress, depression and anxiety. The primary focus of the TUCBH is to care for former military personnel who have separated from active duty.

The Case Manager position at the Tulane University Center for Brain Health will coordinate referrals to resources within the community to further address medical, mental health, behavioral health, and social needs of the patient. When identifying at-risk patients, this position must be able to make critical decisions regarding the patients’ mental/physical health needs to include enhanced social work assistance. Responsibilities may include: clinical and coordination of services management, patient assessment, treatment planning and coordination, multidisciplinary staffing, case management and ongoing program monitoring and development.

VOCATION CASE MANAGER (CRC)

This is a full-time, work-from-home position. The candidate must be located in the Charlotte, NC area due to regular local travel for in-person patient appointments.

Minimum Education: B.S., B.A., in vocational evaluation, special education, behavioral psychology, or related field required. Masters level or advanced study strongly preferred or as required by state law.
Certificates, Licenses, Registrations: C.R.C. or C.D.M.S. eligibility required.
Required Skills and Experience: Meets all vocational case management eligibility requirements of the state/jurisdiction of hire. Computer literacy required.

REHABILITATION SERVICES SUPERVISOR

This position is responsible for providing direct supervision to Vocational Rehabilitation staff including hiring, training, coaching, evaluating, and performing disciplinary action when necessary. The position is responsible for monitoring case management practices, creating a positive work culture, and leading a high performing team. Ideal candidates will possess effective management and supervisory skills, problem-solving and decision making skills; and possess the ability to evaluate data and implement solutions. The position requires ability to collaborate with stakeholders, staff, clients, and community partners. Master’s degree in Rehabilitation Counseling or related field plus one year of experience; CRCC preferred. Multiple positions available.
