Additional Required Information and Forms
You will need to upload additional information to complete your application. CRCC recommends downloading the required forms for the category in which you plan to apply. Completed forms must include the appropriate signatures. When you have the completed forms, simply save them to your computer desktop and upload them as you complete the application.
Incomplete applications will result in a delay in determining eligibility and may delay the applicant’s ability to test in the cycle requested.
STUDENT ENROLLED IN OR A GRADUATE FROM A CACREP-ACCREDITED PROGRAM

Graduate of a Masters or Doctoral Program in a Related Field of Study
Course Syllabi (upload your course syllabi when you apply). Internships and practicum courses may not be used as evidence of completing or teaching courses.
One of the following forms must be submitted with your application:
Employment Experience Forms
Internship Verification Form (must include your supervisor’s signature) 600 clock hours (under CRC supervision; 240 hours direct counseling services to individual with disabilities and must appear in the official transcript.
Educator Employment Verification Form (must include your supervisor’s or employer’s signature) 24 months of full-time employment teaching in a rehabilitation counseling (RC) or clinical rehabilitation counseling (CLRC) program subsequent to earning the master’s or doctoral degree.
Employment and/or Supervision Verification Form (must include your supervisor’s or employer’s signature) 12 months of acceptable work experience under the supervision of a CRC or 24 months of acceptable work experience related to rehabilitation counseling services.
Self-Employment Verification Form (must include the signature of a purchaser of your services) 24 months of full-time employment of acceptable work experience.
